What to Expect
Your first visit at Jamestowne Dental will consist of x-rays and comprehensive examination by Dr. Noble. We ask that you arrive 15 minutes prior to your appointment time to complete any necessary paperwork. Please bring a valid I.D., insurance information, and a current medication list should that apply. We have provided a link below with our new patient form that may be printed and completed prior to your arrival. Call our office today at 317-789-1000 to schedule your new patient appointment!
New Patient Cancellation Policy
We ask that patients provide us with a minimum of 48 hours of notice for an appointment cancellation. New patients who fail their initial appointment without 48 hours notice will not be allowed to be seen at our office.
Insurances Accepted
We accept any insurance that will allow you to choose a provider of your choice and are a networked provider for most insurance companies including but not limited to: Anthem BlueCross & BlueShield, Cigna, Delta, Humana, Medicaid, Metlife, United Concordia, and United Healthcare.
